AQ Training & Testing

The instructions below were developed for use of the AQ in research protocols.

1. Patient Form: The psychologist (or other examiner) should read each item to the patient and record the response. The patient may be provided a copy of the questionnaire so that he/she can follow along. The patient should always be provided with the card showing the response alternatives. If the patient has questions about the meaning of any items, the examiner can provide any necessary clarification by repeating or paraphrasing the items.

2. Family/Significant Other Form: The respondent should be a family member/significant other who knew the patient well prior to the injury and has had frequent contact with the patient since the injury. Parents and spouses are the most appropriate respondents. The exact relationship of the respondent to the patient is recorded. The psychologist (or other examiner) should read each item to the respondent and record the response. The respondent may be provided a copy of the questionnaire so that he/she can follow along. The respondent should always be provided with the card showing the response alternatives. If the respondent has questions about the meaning of any items, the examiner can provide any necessary clarification by repeating or paraphrasing the items.

3. Clinician Form: This form is completed by a psychologist or other clinician working with the patient. Ratings are based on the clinician's judgment of the patient's probable level of premorbid functioning as compared to the patient's current level of functioning. If the clinician also obtains the patient and/or family/significant other data, the Clinician Form should be completed first to avoid bias from previous exposure to the patient and/or family/significant other responses.

All three forms should be completed within 2 working days.
